Required Documentation
The following items are all vital to the underwriting of your loan. We need to have these items within seven (7) business days of filing your application. Numbers 1, 2, and 3 can be filed by fax, with the originals of these documents forwarded with the supporting documentation.
By filing the following documentation with your application, you will definitely help speed up the process for your loan underwriting decision.

- W-2's and/or 1099's for the last two (2) years.
- Copy of last 30 days of pay stubs.
- Three (3) months copies of current bank statements (both checking and savings accounts).
- Divorce Decrees (if applicable)
- Check or money order for $55.00 your credit report made out to: Heritage Plaza Mortgage.
- Landlord's name, address, & phone number for the last two years
- or - the last twelve months of canceled rent checks
- Divorce Decrees (if applicable)
- For Self Employed: Tax returns for the last two (2) years.
- For CHFA or MCC Applications: Tax returns for the last three (3) years.
- For FHA & VA: Social Security Card and Drivers License.
- For VA only: Certificate of Eligibility and DDT-214
